On Monday, August 5^th, from 6:00-7:30pm Fowlerville Athletics will again work with local doctors to offer sports physicals at the High School. The cost is $25 and checks can be made out to Fowlerville Community Schools…take advantage of this opportunity for your high school, junior high and recreation aged athletes! Please download the required forms below or complete them at the school. Athlete and parent signatures are required. Please enter at door 39 (near tennis courts) and be prepared to pick up your child in the bus lot between the HS and JH.

Sports Physicals must have a date of April 15th, 2019 or more current to be eligible for the 2019-20 school year.
